Terms of Service

MyChief is a clinical documentation and decision-support tool for doctors practising in Nigeria. It helps you structure a patient encounter, organise your clinical reasoning, and produce a SOAP note. Some features use artificial intelligence to suggest differentials, propose investigations, draft a treatment plan, transcribe dictation, read a photographed laboratory result, or generate a note.

What MyChief is not

MyChief is not a diagnostic authority, not a prescribing system, not a medical device, not a telemedicine service, and not a hospital management system. It does not practise medicine, and it does not stand between you and your patient.

It also does no patient billing, holds no insurance or claims data, and is not a substitute for your institution’s own medical records where one exists.

To create an account you must:

• be a medical practitioner, or a doctor in training under supervision;
• hold whatever registration or licence your practice requires, and remain in good standing with your regulator;
• be at least 18 years old; and
• be entitled, under your professional obligations and your institution’s rules, to record the patient information you enter.

If you stop meeting any of these, stop using the service and tell us. MyChief is not offered to patients, and a patient should not use it to self-assess.

• An account is personal to you. Do not share your password or let another person work under your account — every note carries your name, and a shared login makes the clinical record untrustworthy.
• Keep your details accurate, particularly your email address.
• You are responsible for what happens under your account. Tell us at privacy@mychief.app if you think it has been accessed by someone else.
• On a shared or hospital computer, sign out rather than closing the tab. MyChief locks itself after a period of inactivity, but that lock is a convenience, not a substitute for signing out.

Every clinical decision made with MyChief is yours. MyChief supports your reasoning; it does not replace it, and it does not share responsibility for it.

The following are terms of this agreement, not advice:

• AI output is a suggestion to be weighed, never an answer to be acted on. Differentials are presented with a level of confidence precisely because the product does not assert a diagnosis. Treat them as prompts to think, not conclusions.
• Verify every drug against the guideline. MyChief refers to dosing charts and national guidelines and does not state doses. Where a plan mentions a medicine, you must confirm the drug, the dose and the route against the Federal Ministry of Health weight-based dosing chart, the Essential Medicines List or the relevant national guideline before prescribing. A paediatric patient must always be dosed by weight.
• Check what the product wrote before you rely on it. Notes generated or drafted by AI, transcripts of dictation, and values read from a photographed result can all be wrong. Read them against the source before finalising a note or acting on a value.
• Flagged critical values are a prompt, not a safety net. MyChief highlights certain results and asks you to acknowledge them. It cannot detect every dangerous value, and acknowledging one does not discharge your own duty to review the result.
• MyChief is not for emergencies. It is a documentation tool. Do not let it sit between a deteriorating patient and the care they need.
• Your professional obligations are unchanged by using it. Your duties to your patient, your regulator and your institution apply exactly as they did before, including your duty to tell a patient that a clinical decision-support tool is being used in their care.

As explained in the Privacy Policy, you are the data controller for the patient records you create, and MyChief processes them on your behalf. That allocation carries obligations that are yours and cannot be passed to us:

• Record only what is clinically necessary, for patients you are actually treating.
• Have whatever lawful basis and consent your practice and the Nigeria Data Protection Act 2023 require, and meet your duty of confidence under the National Health Act 2014.
• Respond to your own patients when they ask to see, correct or delete their records. We cannot do this for you — the records are encrypted with a key we do not hold.
• Do not enter data you have no right to record, and do not upload a document belonging to another practitioner or institution without authority.

Uploading an investigation result — you are the last check

A photograph or scan of a laboratory result usually has the patient’s name printed on it. MyChief gives you tools to take it off before the image is sent to be read: identifiers are found on your device and painted out of the image itself, and you are then shown the exact image that would be sent and asked to approve it.

Those tools propose; you decide. You are responsible for satisfying yourself that the image you approve does not identify your patient. Approving it is the act that sends it.

Five things about that you should know before you rely on it:

• The detector is not a guarantee, and we do not offer it as one. It can miss an identifier — handwriting, an angled or poorly lit photograph, an unusual layout or a low-quality scan all make it less reliable. Read the image you are approving rather than trusting that it has been cleared.
• You can uncover anything it hid. You may remove any hidden area, including one added automatically — this exists so you can reveal a result the detector covered. Whatever you uncover is sent. The review tells you when you have uncovered something; it is then your decision.
• Sometimes nothing is hidden automatically at all. If the detector fails to load, crashes or times out, the review says so and hides nothing. Removing the identifiers is then entirely yours to do, by hand, before you send.
• You can decline to send it. Choosing to save the result without having it read sends nothing to anyone.
• Masking changes only what is sent to be read, never what is stored. The document kept in your patient’s record is your original, encrypted — which is what a clinical record should be, and worth knowing is not a redacted copy.

We cannot do this check for you, and you should not assume we have. MyChief verifies the technical shape of what you send — its format, and that its embedded metadata has been stripped — but it cannot read the picture to judge whether a name is still showing. That is why the review puts the exact image in front of you first: your device is the only place the check can happen, and you are the only person who can make it.

You keep ownership of everything you enter. You grant us only the narrow permission needed to store, transmit and process it so the service can operate — a permission that is in any case bounded by the fact that we cannot read it.

Your records are encrypted on your device with a key derived from your password. When your account is created you are shown a recovery phrase, which is the only other way to unwrap that key.

You are responsible for keeping your recovery phrase. If you lose both your password and your phrase, your records are permanently unreadable — by you, by us, and by anyone else. We have no way to reset, restore or recover them, and we accept no liability for data lost this way.

This is the direct cost of an architecture in which we cannot read your patients’ records. We think it is worth paying; you should make sure you understand it before you put a practice’s worth of clerking into the product. Take an encrypted export from Settings periodically.

The structured clerking workflow is free: every navigable step, from biodata to the note itself. Pro adds the AI features. A limited number of AI-generated SOAP notes is available on the free tier so you can judge the feature before paying for it.

• Price and billing period are shown on the subscription screen in Settings before you pay. Pro is billed monthly in naira.
• Payments are handled by Flutterwave. Your card details are entered on their page and never reach us. Their terms apply to the payment itself.
• Renewal and cancellation. A subscription renews each month until you cancel. You can cancel at any time from Settings; you keep Pro until the end of the period you have paid for, and you are not charged again.
• Refunds. We do not refund part of a period that has already started. If you were charged in error, or the service was materially unavailable for a sustained part of a period you paid for, write to privacy@mychief.app and we will put it right.
• Price changes will be notified at least 30 days before they take effect, and never applied to a period you have already paid for. If you do not accept a new price, cancel before it takes effect.
• Losing Pro does not lose your records. If a subscription lapses, your account reverts to the free tier and everything you have recorded stays exactly where it is.

• Use MyChief for anything other than documenting and reasoning about patients in your own care.
• Present its output to a patient, or to anyone else, as a diagnosis or as a prescription.
• Try to reach another doctor’s data, probe the isolation between accounts, or test the service’s security without our written agreement.
• Scrape, bulk-extract, resell, sublicense or white-label the service, or use it to build a competing product.
• Reverse-engineer the software, except where law expressly permits it despite a term like this one.
• Use automated means to place load on the service, or work around rate limits, capacity limits or the free-tier boundary.
• Upload anything unlawful, or any file you do not have the right to upload.
• Attempt to make an AI feature produce output that ignores its clinical safety constraints.

We will try to keep MyChief available and working, but we do not promise uninterrupted service and there is no service level agreement at this stage. We may change, add or remove features; where a change materially reduces something you rely on, we will give you reasonable notice.

MyChief is an early product, actively developed, and used in a setting where mistakes matter. Clause 4 is written the way it is for that reason. If something looks wrong, tell us at privacy@mychief.app — a report from a doctor in a clinic is worth more to us than any amount of internal testing.

The software, its design, its clinical question banks and everything else we have built remain ours. These terms give you permission to use the service, not ownership of any part of it. Your patient records, notes and uploaded files remain yours (clause 5). MyChief, its name and its logo are our marks; do not use them without permission.

You can delete your account at any time from Settings. Deletion removes your account and the encrypted records attached to it, and it cannot be undone — export first if you want a copy.

We may suspend or end an account that breaches these terms, particularly clause 8, or where we must in order to comply with the law. Except where a breach is serious or ongoing, we will warn you first and give you a chance to put it right. If we end your account other than for a breach by you, we will refund any period you have paid for and not received.

We provide MyChief with reasonable skill and care. Beyond that, and to the extent the law allows, the service is provided as it is: we do not warrant that it will be error-free or uninterrupted, that any AI output will be accurate, complete or clinically appropriate, or that it will meet the requirements of your practice or your institution.

Nothing in this clause limits anything we cannot limit by law, and nothing in it is a reason to rely on the product in place of your own judgement — see clause 4.

We do not exclude or limit our liability for death or personal injury caused by our negligence, for fraud or fraudulent misrepresentation, or for anything else that cannot be excluded or limited under Nigerian law. A clinical product that tried to contract out of exactly the harm it could contribute to would not deserve to be relied on.

Subject to that:

• We are not liable for a clinical decision you make, whether or not MyChief informed it. Responsibility for clinical judgement is yours under clause 4.
• We are not liable for loss of data caused by the loss of your password and recovery phrase (clause 6).
• We are not liable for indirect or consequential loss, loss of profit, loss of goodwill, or loss of anticipated savings.
• Our total liability arising out of or in connection with these terms in any 12-month period is limited to the greater of the amount you paid us in that period and ₦50,000.

You will indemnify us against claims, losses and reasonable costs arising from your breach of these terms, from data you entered that you had no right to enter, or from a claim by a patient or third party relating to care you provided. This does not apply to the extent the claim arises from our own breach or negligence.

We may update these terms. A new version carries a new effective date, and we will tell you in the app or by email where the change is significant. The version you accepted when you created your account is recorded against it. If you do not accept a change, stop using the service and delete your account; continuing to use it after a change takes effect means you accept it.

These terms are governed by the laws of the Federal Republic of Nigeria, and the Nigerian courts have jurisdiction over any dispute arising from them. Before starting proceedings, please write to us — most things are quicker to fix than to argue about.

If any part of these terms is found unenforceable, the rest continues to apply. Our not enforcing a term on one occasion does not waive it.

MyChief is currently operated by its founder and is not yet incorporated. When it is, the registered name and address of the operating entity will be published here and these terms will be reissued with a new version number.
